
Steps to Apply for a Child Passport in Sri Lanka
- Obtain the Application Form:
- You can download the passport application form (K35A) from Above.
- Alternatively, you can collect it in person from the DIE or regional offices.
- Complete the Application Form:
- Ensure all fields are correctly filled out in BLOCK LETTERS.
- Both parents/guardians must sign the application.
- Prepare the Required Documents:
- Original Birth Certificate of the child and a photocopy.
- Copies of parents’ valid National Identity Cards (NICs) or passports.
- Marriage certificate of the parents (if applicable).
- School attendance letter (if the child is attending school).
- Three (3) recent passport-sized color photographs (Studio acknowledgment paper ).
- Consent letter signed by both parents, especially if one parent cannot be present.
- Submit the Application:
- Applications can be submitted at the DIE headquarters in Colombo or regional offices authorized to issue passports.

Is it Necessary to Bring the Child?
- Yes, the child must be present at the time of submission. This is primarily for identity verification and to ensure the child’s physical presence matches the documentation provided.
- In certain cases (e.g., infants), the department may allow exemptions. It is advisable to confirm with the DIE or the respective regional office beforehand.
