Steps to Apply for a Child Passport in Sri Lanka

  1. Obtain the Application Form:
    • You can download the passport application form (K35A) from Above.
    • Alternatively, you can collect it in person from the DIE or regional offices.
  2. Complete the Application Form:
    • Ensure all fields are correctly filled out in BLOCK LETTERS.
    • Both parents/guardians must sign the application.
  3. Prepare the Required Documents:
    • Original Birth Certificate of the child and a photocopy.
    • Copies of parents’ valid National Identity Cards (NICs) or passports.
    • Marriage certificate of the parents (if applicable).
    • School attendance letter (if the child is attending school).
    • Three (3) recent passport-sized color photographs (Studio acknowledgment paper ).
    • Consent letter signed by both parents, especially if one parent cannot be present.
  4. Submit the Application:
    • Applications can be submitted at the DIE headquarters in Colombo or regional offices authorized to issue passports.

Is it Necessary to Bring the Child?

  • Yes, the child must be present at the time of submission. This is primarily for identity verification and to ensure the child’s physical presence matches the documentation provided.
  • In certain cases (e.g., infants), the department may allow exemptions. It is advisable to confirm with the DIE or the respective regional office beforehand.

Fees for Child Passport: Validity period for 3 years

  • Urgent service: Approximately LKR 9,000 (processed on the same day).
